What you will do:

  • Coordinate and provide comprehensive occupational therapy assessments and interventions for clients within the community.
  • Develop, implement and evaluate care plans tailored to each individual’s complex mental health needs.
  • Liaise with the Home Treatment Team, CMHT, and other healthcare providers to ensure a collaborative approach to client care.
  • Maintain accurate and up-to-date case records and documentation, ensuring compliance with professional standards.
  • Promote evidence-based practices, continuously improving therapeutic outcomes for clients.
  • Drive to various community locations, providing care where it is most needed, necessitating a valid driving licence.


To be successful in this role, you must be a registered HCPC professional with evidence of ongoing professional development. Your knowledge of evidence-based interventions for complex mental health conditions will be a crucial asset in delivering high-quality care.
