We are looking to recruit an enthusiastic and motivated individual who has a passion for working with adults, understanding their needs, aspirations and supporting them to enhance their quality of life and promote independence.

As a Social Worker in the Intermediate Care Team, you will work in one of the designated services and undertake assessments and reviews of need under the Care Act 2014, assist with support planning as necessary, and also link people to universal and voluntary services. In addition, you will assist colleagues within other areas of the services as necessary and support with complex assessments, mental capacity, and best interest assessments.

The ability to develop, maintain, and improve relationships with service users and carers is essential in this role, as well as the attributes required to participate in interdisciplinary team working.

Requirements of the Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant frontline experience in Social Services within the UK working as a qualified Social Worker

What You Will Do:
- Conduct assessments and reviews of adults' needs under the Care Act 2014.
- Assist with support planning and guide clients towards universal and voluntary services.
- Support complex assessments, mental capacity evaluations, and best interest decision-making.
- Collaborate effectively within an interdisciplinary team to deliver holistic care.
- Build and maintain strong relationships with service users and carers, enhancing their overall experience and service satisfaction.
