What you will do:
- Manage a diverse caseload of complex patients experiencing severe and enduring major mental illness, delivering personalised care and treatment.
- Work collaboratively within the Lambeth South West Focused Support team to develop and implement comprehensive care plans.
- Utilise your risk management skills to ensure safety and quality of care for all clients.
- Maintain detailed records using the ePJS system and ensure compliance with all relevant guidelines and policies.
- Provide expert guidance and leadership to other members of the team through clinical supervision and support.
