What you will do:
- Complete social care needs assessments to identify and address the needs of adults within the community.
- Develop support plans by working closely with individuals and their families to ensure appropriate and tailored care.
- Commission services and resources to meet identified needs effectively.
- Conduct risk assessments and manage risks by attending MARAM meetings and employing safeguarding practices.
- Engage in Mental Capacity Assessments Best Interest Decisions Court of Protection applications and Deprivation of Liberty Safeguards procedures.
- Contribute to Continuing Healthcare and Decision Support Tool assessments to support individuals with complex health needs.
Requirements of the Social Worker:
- Hold a degree or equivalent qualification in social work.
- Maintain current registration with Social Work England.
- Possess significant frontline experience in UK Social Services as a qualified Social Worker.
- Be a car driver, essential for visiting clients and attending meetings within the community.
