What you will do:
- Lead the Adult Social Care (ASC) representation within the Transfer of Care Hub
- Coordinate and oversee patient-discharge processes to ensure timely and safe transitions
- Collaborate with healthcare professionals to improve discharge planning and processes
- Assess patients' social care needs and develop care plans that address these
- Support and mentor less experienced social workers and healthcare staff
- Maintain accurate and timely records of all cases, ensuring compliance with relevant legislation and regulations
Requirements of the Social Worker:
- Degree or equivalent in social work
- Current Social Work England registration
- Significant frontline experience as a qualified Social Worker in UK Social Services settings
