What you will do:

Conduct comprehensive Care Act 2014 assessments for adult clients to determine their needs effectively.
Draft and implement personalised support plans, offering strategic solutions to improve individual quality of life.
Perform thorough reviews and monitoring of support plans to ensure services are meeting the needs of clients.
Carry out safeguarding enquiries to protect vulnerable adults from abuse or neglect.
Collaborate with multidisciplinary teams and external agencies to coordinate support and services.
Maintain accurate records and provide reports as required, ensuring all documentation is compliant with regulatory standards.
