What You Will Do

Conduct thorough assessments and create tailored care plans in collaboration with other professionals.

Identify social care needs and determine eligibility under the Council’s Fair Access to Care Services criteria.

Commission, arrange, and review services to ensure they meet evolving individual needs.

Lead and contribute to safeguarding investigations, developing and implementing protection plans.

Advocate for service users, promoting independence, inclusion, and overall well-being.

Work within a multidisciplinary team to enhance service quality and outcomes.

Maintain accurate records using information systems for effective case management.
What We’re Looking For

A recognised Social Work qualification and registration with Social Work England.

Strong knowledge of relevant legislation, safeguarding practices, and care frameworks.

Experience in conducting assessments and designing person-centred interventions.
