What you will do:

• Develop and provide specialist dietary plans for patients with Type 1 and Type 2 diabetes
• Work closely with a multidisciplinary team to deliver and manage patient care plans
• Conduct individual assessments and group educational sessions for patients
• Maintain accurate records and documentation in line with HCPC standards
• Travel to various sites as a car driver supporting community-based patients

To be considered, you must be registered with the HCPC and should ideally possess a valid driving licence as travel is required for community visits.
