What you will do:
- Conduct statutory reviews of care and support packages for adults with learning disabilities.
- Assess care and support needs aligned with the Care Act 2014, taking into account risk levels and individual strengths.
- Promote independence and community integration through less restrictive living arrangements.
- Apply the Mental Capacity Act 2005 and relevant guidelines in all practices.
- Undertake mental capacity and risk assessments confidently and competently.
- Safeguard adults by appropriately identifying and responding to concerns.
- Provide professional guidance to individuals and carers to aid informed decision-making.
- Develop and review person-centred support plans reflecting individual goals and well-being.
- Collaborate with families, carers, and providers to ensure needs are effectively met.
