What you will do:

  • Provide specialist care to individuals with learning disabilities and mental health needs through expert clinical assessments in a stepped-care model.
  • Act as a care coordinator by managing access to services, maintaining meticulous records, and ensuring a safe, effective practice in line with HCPC standards.
  • Collaborate with service users, families, carers, and professionals to deliver person-centred, socially inclusive care for those with complex or challenging needs.
  • Use your strong clinical knowledge to support clients with complex needs and challenging behaviours.
  • Confidently participate in multidisciplinary team meetings and contribute to clinical discussions.
  • Apply your skills in risk assessment and care planning effectively.


Qualifications and experience:

  • Registered with HCPC.
  • Experience working within a community mental health setting.
  • Post-qualification experience working with adults with learning disabilities and mental health needs, or solid evidence of transferable skills.
  • Desirable: Experience working in partnership with families and carers, car owner, EPJS experience.
