Key Responsibilities

  • Carry out holistic, strengths-based assessments of clients’ daily living activities.

  • Develop and implement tailored care plans to promote independence and improve quality of life.

  • Liaise with professionals, agencies, and partners to ensure comprehensive support for each client.

  • Recommend and implement equipment and adaptation solutions to meet identified needs.

  • Prioritise and manage a varied caseload effectively, balancing urgency and complexity.

  • Maintain accurate, detailed records using IT systems such as Mosaic.

  • Participate in supervision and professional development activities, ensuring compliance with legislation and safeguarding principles.

  • Travel within the community to carry out home visits and provide support in clients’ own environments.


What We’re Looking For

You should have at least two years of experience in a relevant setting, with a strong understanding of person-centred care. Experience supporting individuals with physical or learning disabilities or mental health conditions is essential. A car driver’s licence is highly desirable to support home visits across the area.
