Social Worker - Joint Reviews Team in Wandsworth UK. Hourly pay rate (Umbrella): £32. Working hours per week: 36, with no option for condensed days and a case load of 15 cases monthly. This assignment is initially for 3 months with the potential for a longer duration. We are seeking a proactive individual to join the Joint Reviews Team, where you will play a vital role in ensuring the highest standards of social care.

What you will do:
- Conduct joint reviews to assess the needs and services required for individuals within the community.
- Develop strong partnerships with families, carers, and other professionals to achieve the best outcomes for service users.
- Prepare and maintain accurate case records and assessments using the system Mosaic.
- Ensure all interventions comply with relevant legislation, guidance, and the council's policies.
- Attend team meetings in person to collaborate with colleagues and contribute to service development.
- Manage a caseload of 15 cases monthly, maintaining high standards of practice.
Requirements:
- Proficiency in using the Mosaic system.
- Minimum of two years of post-qualification experience.
- Ability to conduct face-to-face visits across Richmond and Wandsworth with no expected change to current working from home policies within the next three months.
