Locum Adults Social Worker within Hospital Mental Health in Great Yarmouth UK earning £34 per hour. This is a Full-time role with an Ongoing contract. As an Adults Social Worker, you'll be working with a dedicated team, primarily focusing on The Julian Hospital, which boasts five specialised wards catering to older individuals with complex mental health disorders. Additionally, you will support Laurel Ward at Carlton Court in Lowestoft and be expected to be onsite at least two days per week.

What you will do:
- Collaboratively work with inpatient and outpatient colleagues, community resources, and the care market to support safe patient discharges.
- Undertake Care Act assessments, Continuing Healthcare (CHC) assessments, and Section 117 planning, ensuring comprehensive patient advocacy.
- Lead complex reviews post-discharge in the community to ensure continued patient care and support.
- Engage with patients and multidisciplinary teams to identify and address individual needs.
- Attend essential face-to-face meetings, contributing to decision-making and care planning.
Requirements of the Social Worker:
- Degree or equivalent qualification in social work.
- Current Social Work England registration.
- Significant frontline experience in the UK as a qualified Social Worker within Social Services.
- A good understanding of Mental Health legislation, though desirable, is not essential.
