What you will do:

- Conduct comprehensive assessments of children with disabilities to determine their needs and develop tailored support plans.
- Work closely with families, carers, and professionals to coordinate effective, integrated care packages.
- Attend and contribute to multi-agency meetings to ensure seamless support for children and their families.
- Keep detailed, accurate, and up-to-date records of all interactions and decisions made concerning the children in your care.
- Advocate for the children’s best interests and well-being in line with legal and policy frameworks.

Requirements of the Social Worker:

- A degree or equivalent in social work.
- Current Social Work England registration.
- Extensive experience in frontline Social Services within the UK, working as a qualified social worker with children and disabilities.
