What you will do:

  • Facilitate and support effective, timely hospital discharges across East Sussex, focusing on adults with mental health needs and older adults.
  • Collaborate with Sussex Partnership Foundation Trust, the voluntary sector, and the Adult Team to create safe, person-centred discharge plans.
  • Develop positive relationships with clients, family members, and professionals to ensure an integrated approach to care.
  • Maintain self-motivation and a commitment to your professional development.
  • Meet the travel requirements inherent to this post.


Experience we are looking for:

  • Registration with Social Work England (please include your registration number on your application) and completed the Assessed and Supported Year in Employment (ASYE).
  • Capability to build strong, positive relationships with a diverse set of stakeholders.
  • Self-motivation and a commitment to continuous improvement are essential.
  • Ability to meet the travel requirements of the post effectively.
