What you will do:

- Oversee the referral pathway within the locality CMHT, ensuring the team meets the Waiting Time Standards.
- Formulate integrated needs assessments and monitor care packages, performing regular risk assessments with all involved agencies.
- Work with service users who are difficult to engage in the discharge process, supporting care coordinators to enhance treatment participation and independence.
- Provide ongoing care coordination during transitional phases, ensuring compliance with CPA frameworks and Trust protocols.
- Support clients with interventions under the Care Program Approach, addressing the needs of black and minority ethnic users with a recovery-oriented, client-centred approach.
- Involve families and carers in treatment plans, facilitating services within the guidelines of the Children & Families' Act and Care Act.
- Develop good communication links between multidisciplinary teams and referring agencies for better service delivery.

Essential skills / experience:

- A robust understanding of service eligibility criteria and the ability to support clinicians in this area.
- Excellent team management and leadership skills, fostering good communication practices across multiple agencies.
- Proficient in engaging and supporting service users and their families within treatment plans.
