What you will do:

• Conduct strength-based assessments and develop person-centred support plans to meet the individual needs of adult service users.

• Implement long-term support arrangements and regularly review cases to ensure the continued well-being of clients with complex needs in the community.

• Perform Mental Capacity Assessments and engage in Court of Protection work as required.

• Manage an allocated caseload within the framework of performance management, quality assurance, and budgetary constraints to maintain high service standards.

• Act as a team member, providing a personalised brokerage service to adult service users, carers, and their families.
