What you will do:

• Take charge of assessing patients and planning for their discharge within a strict 3 to 6-day timeline, ensuring a swift and safe transition from hospital to home.

• Conduct safeguarding evaluations for patients who may have experienced neglect or abuse during their stay, ensuring their safety and well-being.

• Participate in duty rotations once a week, contributing to the smooth operation of the social work team.

• Attend meetings and complete assessments at Wexham Park Hospital, where your travel arrangements are facilitated by a parking permit.

• Utilise the LAS system to streamline your work processes, particularly if you have prior experience with this tool.
