We currently have an exciting opportunity for a Band 6 Community Psychiatric Nurse (CPN) to join the Community Mental Health Team within Older People in Woking, earning £27 hourly. This is an ongoing position and offers both full-time and part-time shifts Monday to Friday, 9am to 5pm. What you will do:

• Assess, plan, implement, and evaluate care for older adults experiencing mental health issues.

• Work closely with multidisciplinary teams to support the mental well-being of patients.

• Provide support and education to families and caregivers, helping them understand the patient's condition and care plan.

• Participate in team meetings, contributing to case discussions and patient assessments.

• Maintain comprehensive and accurate patient records using SystmOne.

• Engage in continuous professional development to keep up-to-date with best practices.
