What you will do:

Contribute to the Adult Health and Social Care Transformation programme, improving outcomes for citizens.

Complete strengths-based, outcome-focused assessments, supporting both adults and (where applicable) children.

Oversee case management, ensuring care plans are delivered and reviewed appropriately.

Participate in continuous professional development through reflective practice, supervision, and learning.

Maintain compliance with ethical frameworks and legislative guidelines.

Manage a busy caseload, ensuring effective and timely service delivery.

Collaborate with community organisations and statutory partners for integrated care planning.
Who we’re looking for:
You should have at least three years of post-qualified experience in permanent roles within local authorities. The ideal candidate will be adaptable, confident, and solution-focused, with:

Proven ability to handle difficult conversations, especially relating to Direct Payment Misuse.

Experience in Continuing Health Care (CHC) and strengths-based working.

A track record of success in review teams or fast-paced social work settings.

Flexibility to contribute to different teams and projects as needed.

Willingness to work onsite 2–3 days per week.

Familiarity with Liquid Logic software (preferred).
