What you will do:

- Facilitate and manage safe hospital discharges for service users, enabling a smooth transition back home.
- Conduct comprehensive assessments of individual needs, focusing on functional abilities, moving and handling, as well as housing.
- Prescribe suitable equipment and recommend minor home adaptations to support individuals in maintaining independence.
- Collaborate closely with staff to ensure the best outcomes for service users, fostering a team-oriented approach to address various rehabilitation needs.

To excel in this role, being registered with the Health and Care Professions Council (HCPC) is essential, along with possessing a full driving licence to provide the flexibility needed to deliver services effectively across different locations.
