We are seeking a dedicated and experienced Social Worker to join our dynamic Adults Locality Team in Oldbury. Key Responsibilities:

• Manage a varied caseload involving adults with complex and diverse needs.

• Undertake Care Act 2014 assessments and reviews to deliver personalised support plans.

• Conduct triage calls, manage referrals, and deliver timely interventions to support community independence.

• Complete a minimum of three case-related tasks per week to maintain service efficiency.

• Attend team meetings every other Friday to support shared learning and service alignment.

• Contribute to waiting list reduction and case review initiatives to enhance access to support.

Requirements:

• Degree in Social Work (DipSW, CQSW, or equivalent)

• Social Work England registration

• Strong working knowledge of the Care Act 2014, Mental Capacity Act 2005, Community DoLS/LPS, Safeguarding, and Continuing Health Care

• Experience working in a fast-paced, high-volume adult services setting

• Confident communicator with excellent organisational and documentation skills
