Locum Social Worker within Adults Intermediate Care Team in Warrington, earning £30 Hourly, Ongoing. What you will do:
- Work collaboratively with individuals to assess their needs and develop personalised care plans focused on promoting recovery and independence.
- Co-ordinate the delivery of intermediate care services, ensuring a seamless transition for clients returning home from hospital or requiring additional support.
- Evaluate and monitor client progress, adapting care plans as necessary to achieve the best outcomes for service users.
- Participate in multidisciplinary meetings to share insights and strategies for enhancing service delivery.
- Assist in delivering technology-enabled care and arranging equipment and adaptations to support clients in their own homes.
