What you will do:

- Assessment: Conduct comprehensive assessments of individuals’ physical, cognitive, and emotional needs to identify barriers to daily activities and determine appropriate interventions.
- Care Planning: Develop and implement personalised care plans to improve clients’ functional abilities, independence, and quality of life. Collaborate with healthcare teams to ensure holistic and coordinated care.
- Therapeutic Intervention: Deliver evidence-based interventions to support clients in achieving their goals. This may include teaching techniques for tasks like dressing, cooking, bathing, or working, and recommending adaptive equipment.
- Rehabilitation: Work with individuals recovering from illness, surgery, or injury to restore function, mobility, and independence. Support clients with neurological conditions, stroke rehabilitation, or orthopaedic recovery.
- Education: Provide education and training to clients and their families on managing daily activities, using assistive technologies, and maintaining health and well-being at home.
- Care Coordination: Liaise with other healthcare professionals (doctors, physiotherapists, social workers, etc.) to ensure a multidisciplinary approach to patient care. Maintain accurate and up-to-date case notes and treatment records.

To thrive in this role, you'll need to be HCPC registered and ideally have a driving licence to access various locations easily.
