Key Responsibilities

Undertake assessments and care planning for children with disabilities, ensuring access to appropriate support and services.

Work closely with families, carers, and partner agencies to develop holistic, strengths-based plans.

Provide professional advice and guidance to families, empowering them to navigate services effectively.

Maintain accurate and comprehensive case records in line with statutory requirements and best practice.

Act as an advocate for children, ensuring their voices are heard and their needs prioritised in all decision-making.

Contribute to multi-agency working, promoting collaboration and consistency in support delivery.
Essential Requirements

Qualified Social Worker (BA/MA Social Work, DipSW or equivalent).

Current registration with Social Work England.

Proven experience in children’s services, ideally within a disabilities or complex needs team.

Strong knowledge of safeguarding, disability legislation, and relevant statutory frameworks.

Excellent communication, assessment, and case management skills.
