Key Responsibilities

• Undertake assessments and care planning for children with disabilities, ensuring access to appropriate support and services.

• Work closely with families, carers, and partner agencies to develop holistic, strengths-based plans.

• Provide professional advice and guidance to families, empowering them to navigate services effectively.

• Maintain accurate and comprehensive case records in line with statutory requirements and best practice.

• Act as an advocate for children, ensuring their voices are heard and their needs prioritised in all decision-making.

• Contribute to multi-agency working, promoting collaboration and consistency in support delivery.

Essential Requirements

• Qualified Social Worker (BA/MA Social Work, DipSW or equivalent).

• Current registration with Social Work England.

• Proven experience in children’s services, ideally within a disabilities or complex needs team.

• Strong knowledge of safeguarding, disability legislation, and relevant statutory frameworks.

• Excellent communication, assessment, and case management skills.
