Working full time, 37.5 hours per week, Monday through Friday, you'll play a vital role in providing medicines optimisation services and support to both GPs and the dispensary team at HMP Channing Wood and the Devon Cluster.

What you will do:

- Provide support to patients with medicine queries and concerns, ensuring they receive personalised and effective care.
- Conduct medication review clinics to optimise patient treatment plans.
- Stratify risk with high-risk medicines, working diligently to reduce potential risks for patients.
- Collaborate with the health and wellbeing team to manage medicine-related risks and implement improvements for patients with long-term conditions.
- Lead the development of new services around innovative medicines or NICE guidance.
- Ensure compliance with CQC standards, providing leadership and support when necessary.
- Work closely with the Specialist Prison Services Pharmacist to develop and enforce the formulary.
- Support the audit process to improve prescribing practices.
