Key Responsibilities

• Assess and address social care needs of adults experiencing mental health conditions, including those with co-existing substance misuse issues.

• Conduct comprehensive crisis and risk assessments to ensure the safety and wellbeing of individuals and their carers.

• Design personalised support plans in line with Care Act eligibility, budgetary guidelines, and person-centred care principles.

• Encourage self-directed support through direct payments to empower independence and autonomy.

• Carry out safeguarding duties, identifying risks of significant harm and taking necessary protective measures while respecting individual rights.

• Build effective, collaborative partnerships with public, voluntary, and private sector agencies to ensure joined-up, holistic care.

• Maintain accurate and timely case recordings using Liquid Logic.

• Be open to undertaking AMHP training and supporting the AMHP rota where required.

Essential Requirements

• Degree or equivalent qualification in social work (e.g. CQSW, CSS, DipSW)

• Current registration with Social Work England

• Minimum 5 years’ post-qualification experience in CMHT and Adult Social Care

• Familiarity with Liquid Logic

• Driving licence preferred
