Qualified Social Worker - Adults Transition Team - Ashton-Under-Lyne - £35 Hourly - Full Time (36 hours per week, Monday to Friday with the occasional Saturday)

Here's a fantastic opportunity to work as a Qualified Social Worker within the Adults Transition Team in Ashton-Under-Lyne. This is an ongoing position with a pay rate of £35 per hour. You'll be joining a dedicated team, working full-time to help young individuals transition smoothly from children's to adult services. What you will do:

  • Collaborate across various disciplines to continuously promote a needs-based approach to individuals transitioning to adult services.
  • Develop clear and actionable support plans following Care Act assessments, including completing funding applications and reports for court, as necessary.
  • Engage meaningfully with individuals and families, fostering partnerships for effective service delivery.
  • Support carers and families through comprehensive Care Act provisions.
  • Work with children transitioning to Adult Mental Health services and be knowledgeable about SEND and EHCPs.
  • Provide support within a 6-day service framework, mainly operating Monday to Friday with occasional Saturdays, requiring flexibility to plan interventions effectively.
