What you will do:

• Conduct initial and discharge assessments for Pulmonary Rehabilitation, ensuring personalised care plans.
• Lead and manage Pulmonary Rehabilitation classes, providing essential education to patients in a community setting.
• Perform comprehensive respiratory assessments on patients, including those with confirmed conditions like COPD and interstitial lung disease, and develop appropriate physiotherapy treatment plans.
• Work collaboratively within a multidisciplinary team to ensure holistic patient care.
• Continuously update your knowledge and expertise in pulmonary rehabilitation through ongoing professional development.

To make the most of this dynamic role, you will need to be HCPC registered and possess a valid driving licence, as community visits are a key component of the job.
