• Conduct effective Care Act assessments, care planning, and care management services for adults with high mental health social needs, offering joint assessments as required.
• Collaborate closely with a diverse range of team members to ensure a comprehensive and empathetic assessment of service user needs.
• Apply your mental health experience to support service users and facilitate positive outcomes in complex situations.
