What you will do:

- Safeguard and promote the wellbeing of children with disabilities through high-quality social work practice
- Conduct assessments aligned with the Social Services and Well-Being Act (Wales) 2014
- Develop and review care and support plans, ensuring they are monitored effectively
- Undertake Mental Capacity Assessments as per the Mental Capacity Act 2005
- Assess and mitigate risks promoting coproduction, independence, and choice
- Collaborate with various agencies and work within a multidisciplinary framework
- Maintain accurate and up-to-date electronic records in line with departmental policy
- Travel independently throughout the service area to meet the needs of service users
- Embrace remote working with one in-office day per week, providing a balance between flexibility and team cohesion
