Role Responsibilities:

  • Undertake varied casework including support planning, commissioning care, conducting reviews, and mental capacity assessments.

  • Facilitate solution-focused, strengths-based conversations, including Conversation 3 discussions to assist young people transitioning to adult services.

  • Participate in multi-disciplinary meetings, including best interest meetings, EHCP reviews, Section 117 reviews, and CHC checklist assessments.

  • Work closely with senior practitioners and team managers, drawing upon their support and professional guidance.

  • Act as a dedicated advocate for young adults, ensuring their voice, wellbeing, and goals are prioritised in every care decision.


Candidate Requirements:

  • A minimum of 2 years’ post-qualification experience in social care, ideally within a transitions or related service.

  • Strong case management skills and the ability to work effectively in a fast-paced, evolving environment.

  • Passionate about achieving positive outcomes for young people and committed to empowering their independence.

  • Reflective, proactive, and capable of contributing meaningfully within a collaborative team structure.
