Role Responsibilities:

• Deliver holistic assessments, care planning, and reviews under the Care Programme Approach.

• Work collaboratively with service users, families, and professionals to establish robust support networks.

• Advocate for individuals with complex mental health needs, promoting autonomy and recovery.

• Apply legislation such as the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983, and Human Rights Act 1998.

• Fulfil statutory social work functions, including safeguarding responsibilities.

• Promote early intervention to reduce the risk of crisis through effective signposting and support.

• Participate in multi-disciplinary meetings, contributing social work insights and expertise.

• Encourage a strengths-based, solutions-focused approach to drive meaningful change.

Candidate Profile:

• Qualified Social Worker or Approved Mental Health Professional (AMHP).

• Strong ethical and legal understanding with sound decision-making abilities.
