Key Responsibilities:

• Deliver assessments and care planning in accordance with the Care Act 2014, focusing on prevention, support, and safeguarding.

• Apply a Strength-Based Approach, promoting dignity, cultural sensitivity, and clear communication in all aspects of support.

• Foster effective partnerships with services such as Technology Enabled Care, Reablement Services, and Review Teams for holistic service delivery.

• Propose and implement creative solutions that enhance integration between Adult Social Care and Health Services.

• Identify safeguarding concerns and respond appropriately, including conducting investigations and developing protection plans.

• Chair Safeguarding Meetings and coordinate or lead multi-disciplinary team (MDT) meetings to ensure effective service planning.

• Maintain high standards of electronic case recording, ensuring accuracy and compliance with council procedures and statutory guidance.

• Supervise, support, and develop staff, ensuring consistent quality in service delivery and professional development.

• Assist Team Managers and Service Managers with operational matters to ensure continuity of support for both staff and service users.

• Undertake specialist duties aligned to your area of expertise, in accordance with current policy and practice guidelines.
