Key Responsibilities:

Deliver assessments and care planning in accordance with the Care Act 2014, focusing on prevention, support, and safeguarding.

Apply a Strength-Based Approach, promoting dignity, cultural sensitivity, and clear communication in all aspects of support.

Foster effective partnerships with services such as Technology Enabled Care, Reablement Services, and Review Teams for holistic service delivery.

Propose and implement creative solutions that enhance integration between Adult Social Care and Health Services.

Identify safeguarding concerns and respond appropriately, including conducting investigations and developing protection plans.

Chair Safeguarding Meetings and coordinate or lead multi-disciplinary team (MDT) meetings to ensure effective service planning.

Maintain high standards of electronic case recording, ensuring accuracy and compliance with council procedures and statutory guidance.

Supervise, support, and develop staff, ensuring consistent quality in service delivery and professional development.

Assist Team Managers and Service Managers with operational matters to ensure continuity of support for both staff and service users.

Undertake specialist duties aligned to your area of expertise, in accordance with current policy and practice guidelines.
