This is an exciting opportunity for a skilled Social Worker to join the Hospital Discharge Team in Ealing. What you will do:

- Manage a caseload of patients, collaborating with a multidisciplinary team for comprehensive care.
- Provide guidance on social care processes, ensuring patients and families understand available support options.
- Proactively engage in discharge planning, anticipating obstacles and addressing them early in the process.
- Connect patients and carers with support services related to benefits, housing, and more.
- Coordinate with external agencies to solve issues such as homelessness or lack of public funds.
- Contribute to the efficient management of patient flow across inpatient pathways, maintaining high quality care.
- Mentor the team on navigating social care roles and the implications of The Care Act.
- Support complex stroke case discharges with the Neuronavigator and other teams.
- Deliver clinical expertise to external agencies as required.
