Social Worker for Adults Intermediate & Frailty Team in Halton UK - £35 per hour, Hourly. Ongoing contract.

What you will do:
- Work closely with older adults and individuals with frailty and complex needs.
- Perform strengths-based assessments and develop effective care and support plans.
- Collaborate with multi-disciplinary partners including health professionals, hospital teams, community services, and voluntary organisations.
- Support safe hospital discharges and prevent avoidable admissions.
- Undertake Mental Capacity Act assessments and safeguarding enquiries where required.
- Promote independence, resilience, and high-quality outcomes for adults and their families.

We are seeking a practitioner who is:
- A qualified Social Worker with SWE registration.
- Experienced within adult social care, ideally with knowledge of frailty, hospital discharge, or intermediate care pathways.
- Confident in risk assessments, safeguarding practices, and person-centred planning.
- Skilled at working in fast-paced, integrated environments.
- Enthusiastic about delivering excellent care and improving the lives of Halton residents.
