What you will do:

- Conduct comprehensive assessments of adult wellbeing and needs with an emphasis on early intervention and prevention.
- Co-produce detailed support plans with individuals, focusing on their strengths and desired outcomes.
- Undertake mental capacity assessments, best interest assessments, and necessary applications to the Court of Protection.
- Ensure safeguarding is central to your role, addressing any risk of abuse or neglect for vulnerable adults.
- Perform risk assessments with a proportionate and least restrictive approach.
- Collaborate with multiple professionals to coordinate support for adults with complex needs.
- Manage a caseload efficiently, contributing to the duty rota, including occasional weekends.
- Build a team culture focused on the voice of lived experience.
- Engage in continuous learning and contribute to the team's development by supporting colleagues and students.
- Maintain accurate records and comply with departmental policies, ensuring consistent high-quality service delivery.

About you:

Your essential qualifications:
- Social Work degree CQSW DIPSW CSS
- Registration with Social Work England
- Full Driving Licence

Your essential experience:
- Experience in social or health care settings
- Proficient in completing assessments and support plans
- Skilled in communication with the public and stakeholders
- Familiar with joint work with other agencies

Your essential skills:
- Excellent written and analytical skills
- Strong communication skills across varied audiences
- Expertise in needs and risk assessments
- Capable of working within a multi-disciplinary framework
- Competence in IT tools for accurate data maintenance

Your essential knowledge:
- Insight into Adult Services legislative and social policy contexts
- Familiarity with the Care Act 2014 and relevant safeguarding acts
- Understanding social work theories and evidence-based applications

Your essential qualities:
