What You Will Do

• Apply your expertise within the framework of the Care Act 2014, focusing on improving well-being and promoting independence for adults and carers.

• Manage a varied and reasonable caseload using a strengths-based, person-centred approach to deliver high-quality assessments, support plans, and reviews.

• Engage in reflective supervision and contribute to continuous service improvement through learning and innovation.

• Provide expert advice, guidance, and advocacy to individuals, families, and partner professionals.

• Maintain accurate and confidential case records in line with council policies and statutory requirements.

• Build strong working relationships with colleagues and partner agencies to ensure integrated and effective care pathways.

• Champion safeguarding principles, exercising professional curiosity and accountability in every case.

• Embrace lifelong learning and actively participate in CPD programmes to maintain and enhance your professional capabilities.
