What You Will Do

Apply your expertise within the framework of the Care Act 2014, focusing on improving well-being and promoting independence for adults and carers.

Manage a varied and reasonable caseload using a strengths-based, person-centred approach to deliver high-quality assessments, support plans, and reviews.

Engage in reflective supervision and contribute to continuous service improvement through learning and innovation.

Provide expert advice, guidance, and advocacy to individuals, families, and partner professionals.

Maintain accurate and confidential case records in line with council policies and statutory requirements.

Build strong working relationships with colleagues and partner agencies to ensure integrated and effective care pathways.
