What you will do:

- Undertake Care Act–compliant comprehensive OT assessments for adults aged 18+ with disabilities
- Apply sound clinical reasoning to assess risk, promote independence, and support safe outcomes
- Prescribe simple and complex equipment, completing associated risk assessments
- Complete Manual Handling assessments while providing clear moving and handling recommendations
- Assess for and recommend major adaptations in line with statutory criteria
- Refer for and contribute to Disabled Facilities Grant processes where appropriate
- Participate in duty triaging on a rota basis, screening referrals, responding to urgent matters, and prioritising cases appropriately
- Maintain accurate, timely, and professional case recording
- Work collaboratively with service users, carers, health professionals, housing teams, and partner agencies

Requirements:

- Must be HCPC registered
- Need to be a car driver to navigate the community efficiently
