What you will do

Care Act Assessments: Conduct assessments in line with the Care Act 2014 to identify needs and develop tailored care plans.
Aftercare Responsibilities: Undertake Section 117 Aftercare duties to support recovery and long-term wellbeing.
Safeguarding: Carry out Adult Safeguarding work to protect vulnerable individuals.
Strengths-Based Practice: Promote independence through strengths-based approaches and personalised interventions.
Multidisciplinary Working: Participate in multi-agency meetings with health and social care professionals.
Service Delivery: Provide high-quality, person-centred support to adults with mental health needs.

Qualifications / Requirements

Qualified Social Worker with Social Work England registration.
Experience within Adult Social Care, ideally within a CMHT setting.
Strong knowledge of the Care Act 2014 and safeguarding procedures.
Experience supporting adults with mental health needs.
Ability to work effectively within multidisciplinary teams.
Excellent communication, assessment, and organisational skills.
