We have an excellent opportunity for an experienced Occupational Therapist to join a Dementia Intensive Support Team in Bury St Edmunds. What you will do:
- Provide urgent responses to individuals with cognitive impairments and their carers, both via telephone and in person.
- Manage caseloads efficiently, ensuring patient needs are met promptly and effectively.
- Use your assessment skills, including risk assessment, to tailor support and intervention plans.
- Work collaboratively with other stakeholders, communicating decisions to the multidisciplinary team (MDT).
- Partake in MDT discussions, reflective practice, and supervision, contributing to a team-focused approach.
- Competently apply the Mental Capacity Act in practice, ensuring ethical standards are consistently maintained.
- Triage cases to determine the needs of the family system and document decisions accurately.
Requirements:
- HCPC registration is essential.
- Minimum of 2 years of experience working with people with dementia and their carers.
- PMVA training is required.
- A car driver is preferable to facilitate community visits and support.
